Service-Learning Proposal Description

1.

Briefly describe the project and the community need to be addressed.

Answer Our volunteer Navigators assess individuals' social needs and connect them with community resources to address underlying social needs that can negatively impact their health.

What is the mission of the agency or organization?

Answer Marshfield Clinic Health System healthcare knows your health is affected by your community more than by the care they can provide you within the clinic walls. Individuals are screened during health care visits to identify areas of need, such as food, transportation, clothing and utilities. Our Community Connections Team (CCT) enlists UW volunteers to connect individuals with local resources to meet their needs and helps individuals and families improve their health and ultimately, their quality of life.

What skills are required for this project? 

Answer Volunteer Navigators should have willingness to communicate with individuals and provide them local resources to meet their needs and help individuals and families improve their health and quality of life.

4.

What are the objectives for the student learning and service?

Answer While volunteering for CCT you will gain valuable communication skills, critical thinking, and a deeper understanding of the broader side of healthcare.

5.

What are the availability requirements for the student?

Answer We ask for a three-hour shift one day a week. Due to onboarding and training we do ask for a two semester commitment.
