Service-Learning Proposal Description


Briefly describe the project and the community to be addressed.
Answer Individuals with a desire to serve are invited to join our team at Allina Hospice & Palliative Care! Our volunteers impact the lives of our patients, their family members and caregivers by providing a variety of services such as, but not limited to: companionship; helping with errands; providing respite for caregivers; meal prep; light housework and transportation. Patients reside in their own homes or residential care facilities (including assisted living facilities) in western Wisconsin and the greater Twin Cities metro area.


What is the mission of the agency or organization?
Answer The mission of Allina Health Hospice is to meet the physical, emotional and spiritual needs of patients at end of life. We provide support and information not only to our patients, but to the patient's family members and caregivers. Our volunteers are integral members of our hospice teams.


What skills are required for this project?
Answer We are looking for individuals with a desire to serve and who are good listeners. We also invite those in who welcome calmness and quiet; we will help you find your perfect fit! Individuals will need to be able to respect personal boundaries and respect confidentiality.


What are the objectives for the student learning and service?
Answer -Complete in person interview -Pass required state background study -Complete Health Assessment -Complete 5 hours of online training -Complete a 1x, 3 hour group training session in St. Paul -Gain knowledge of hospice/healthcare/social/intergrative services -Commit to 30 plus hours of service once you have completed training


What are the availability requirements for the student?
Answer 1-3 hours of time per week (when assigned to a patient)
