Service-Learning Proposal Description

1.

Briefly describe the project and the community need to be addressed.

Answer St. Croix Hospice is looking for individuals to make companionship visits to our patients. As a companionship volunteer you can enrich the lives of terminally ill patients in your community. Visits may involve playing games, reading a book, watching a movie, a walk or just enjoyable conversation. Hours are extremely flexible. Become a friend/companion to a patient when they really need it most! If you have a little extra time and are looking for something rewarding to do, look no further. When assigned to a patient, we will review the details of the patient’s life before we introduce you to them. We provide you with all the training and any resources you may need.

What is the mission of the agency or organization?

Answer Not all hospices are the same. As hospice care providers, St. Croix Hospice listens to patients and families to understand their individual wishes for the end-of-life care journey. With a focus on comfort and quality of life, our hospice teams provide care with dignity and respect.

Answer Organizational and people skills.

What are the objectives for the student learning and service?

Answer Learn about the end of life process and the people that go through it.

What are the availability requirements for the student?

Answer Very flexible schedules.

Answer Interview, background check, TB test and online training (which is used towards hours for student) Very flexible schedule and a wide array of different tasks to choose from.
