Service-Learning Proposal Description

1.

Briefly describe the project and the community need to be addressed.

Answer We have a variety of hospice volunteer phone call opportunities available. 1. Bereaved Calls - Volunteers assist the Bereavement Team on making 8-month check in calls to bereaved families that requested bereavement services after their loved one passes away. This is very flexible and can be done anytime within a two-week period from your own home. 2. Tuck-In Calls - Volunteers will get a list of home patients to contact between the hours of 9 am - 1pm on Thursdays to see if families need anything for the weekend. This can be done at your own home. 3. Welcome Calls - Volunteers make calls to a new patient's family member to verify contact and demographic information. This can be done at your own home. Training is provided.

What is the mission of the agency or organization?

Answer Not all hospices are the same. As hospice care providers, St. Croix Hospice listens to patients and families to understand their individual wishes for the end-of-life care journey. With a focus on comfort and quality of life, our hospice teams provide care with dignity and respect.

Answer Organizational skills, phone etiquette

Answer End of life process and the people that go through it.

What are the availability requirements for the student?

Answer Very flexible schedule.

Answer Interview, background check, TB test and online training (which is used towards hours for student) Very flexible schedule and a wide array of different tasks to choose from.
